Your answers to the eValuation Form found below will give our certified ergonomic professionals the information they need to provide an ergonomics assessment of your office environment. This assessment will outline workplace factors which may need to be modified to reduce or eliminate at-risk postures. It will also identify improvements and modifications you can make to your work habits and equipment which should lead to improved comfort while performing your tasks.
The form should take less than 15 minutes to complete. The eValuation will have enhanced value if you have someone in your office take (3) digital photos of you at your workstation to show the interface you have with your working components. Photos should be taken to include side views which include you sitting in your office chair, working at your keyboard/mouse set-up, and the general workplace area. (These photos will be used for ergonomic analysis and will ONLY be shared with those your designate with your prior approval.) Please attach the digital images (800KB or smaller) to the end of the form before submitting your eValuation.
After you complete the eValuation form and click the submit button, you will be redirected to the Digital Evaluation product page of the OnLine Store. At that point, you will need to click the "Buy Now" button to place your order which authorizes us to perform this assessment for you. The fee for this digital assessment is US $95.00.
Within 48 hours, our certified ergonomics evaluators will complete an assessment report that will identify the posture stressors at your workplace, which actions can be implemented to provide safer and more comfortable working biomechanics, and recommendations of any ergonomic equipment that would be beneficial.
